By booking and attending a consultation with Clarity Telehealth, you acknowledge and agree to the following:

 

1. Nature of Telehealth

Telehealth involves the delivery of medical services via phone or video consultation.

 

2. Limitations of Telehealth

You understand that:

  • A physical examination is not possible

  • Diagnosis may be limited

  • You may be advised to seek in-person medical care

 

3. Clinical Decision-Making

You acknowledge that:

  • Prescriptions, referrals, and medical certificates are not guaranteed

  • All decisions are based on clinical judgement and safety

 

4. Weight Loss & Prescribing

Where applicable, you understand that:

  • Weight loss medications are prescribed only when clinically appropriate

  • Risks, benefits, and alternatives will be discussed

  • Ongoing monitoring or follow-up may be required

 

5. Privacy and Security

Reasonable steps are taken to ensure secure communication. However, no electronic system is entirely risk-free.

 

6. Emergency Situations

Telehealth is not suitable for emergencies.
If you experience severe symptoms, call 000 or attend the nearest emergency department.

 

7. Consent

By proceeding with the consultation, you:
